A governance and survey-preparation console for Directors of Nursing, Activity Directors and administrators of memory care and long-term care communities.
Memory care programming is judged on whether each resident’s activities fit that resident. Cognicopia gives your team three things a surveyor can see on paper: sheets built from the resident’s own life, a dated page for the chart, and print that survives a busy copier room.
Pillar 1 · Clinical
Person-centered, non-pharmacological engagement
Built from the resident. Work, hobbies, hometown, the decades they grew up in and the topics to avoid shape every sheet.
Matched to the stage. Type is never under 18 pt bold (early and middle) or 24 pt bold (late), and grows with a large-print or extra-large vision setting; late-stage pages carry one thing to look at, with nothing else on the sheet.
Adult dignity. A filter on every page stops quizzing, scoring, baby talk and child-directed words. Subjects are tools, gardens, music, the kitchen and the radio.
Evening support. A calm, low-demand sheet is available for the late-afternoon and sundowning hours, with a one-line tip for the person sitting beside the resident.
Pillar 2 · Operational
Copier-ready, zero staff prep
Pure black, vector geometry. Lines and type print as 100% black (#000000) on white, with no grey fills to dither or smear across repeated copy generations. Results vary by machine; Tab 3 tests yours.
Clipboard and binder safe. Sheets keep a binding gutter on the left (0.75 in on Cognicopia pages; Tab 3 tests that your clipboard clears 0.65 in) and nothing within 0.5 in of any edge.
Nothing to prepare. No cutting, folding or assembly. Choose the resident, press print, hand out the sheets.
Standard stock. US Letter, single-sided, on the 20 lb bond a facility already stocks.
Pillar 3 · Governance
Independent publisher, local data
One accountable team. Cognicopia reviews the catalog and answers facility questions directly at cognicopia@outlook.com.
Nothing leaves the computer. The tools run in the browser. There is no server, account or cloud copy of resident details.
Encrypted at rest. Saved details are sealed on the computer (AES-256-GCM) and can be locked with a passphrase. Printed sheets, shared computers and passphrase handling stay under your own privacy program.
Procurement-ready. Net 30 invoicing, a vendor information sheet and a W-9 (Tab 4).

CMS Tag F679 42 CFR §483.24(c)
An ongoing program of activities, based on the comprehensive assessment, the care plan and each resident’s preferences, that supports their interests and their physical, mental and psychosocial well-being.
A daily, individualized, non-pharmacological activity built from the resident’s own history and matched to their stage: evidence that the program is person-centered and ongoing, not generic.
The assessment, the care plan, the activity calendar, observation of participation and resident and family interviews.
MDS 3.0 Section F and Section Q
Section F records the resident’s preferences for customary routine and activities. Section Q records participation in assessment and goal setting.
A dated, completed sheet with the resident’s name, the date and a staff note is a physical artifact the interdisciplinary team can bring to a care-plan review.
Every assessment answer and every MDS code. A sheet informs the clinician; it never replaces the clinician’s judgment.
Care planning F656 · §483.21(b)
A person-centered care plan that is put into practice and revised as the resident changes.
Stage-matched sheets (early, middle, late) make it easy to step an activity up or down, and the page itself shows what level was used.
Writing, approving and revising the plan.
Privacy practices HIPAA privacy program
Reasonable safeguards for resident information, decided and documented by the community.
No transmission of resident details, no cloud copy, encryption on the computer and an optional passphrase: a design that supports those practices.
Policies, training, workstation security, how printouts are stored and who may use the computer.
This table paraphrases the regulations to explain how the sheets are used. It is not legal advice, and no agency has endorsed, reviewed or certified Cognicopia. Check the current State Operations Manual (Appendix PP) and your state agency’s guidance for the exact language.
Operational durability, in plain terms
Pure #000000 vector geometry. Because pages use only black and white, a copier has no tint to screen into dots. Edges stay crisp on the tenth copy of a copy.
Heavier lines for unsteady hands. The thinnest line Cognicopia prints is 2.25 pt (early), 3 pt (middle) or 4.5 pt (late). Coloring outlines are 3 pt or heavier.
A gutter that clips. The binding edge stays clear so a clipboard or binder ring never covers a word.
Built for high-volume copy rooms. Made for the monochrome laser copiers common in care communities (Ricoh, Xerox, Canon, HP and similar). Run the Tab 3 sheet on each machine you use.
Publisher’s commitment
Every Cognicopia resource is published by Cognicopia, which commits to clinical dignity, print precision and responsive facility support. If an activity packet does not engage your residents or meet your community’s clinical requirements, the publisher will provide a custom adaptation or a full institutional refund. Reach Cognicopia directly at cognicopia@outlook.com.
A six-point check of your own evidence against what surveyors look for in activities programming (CMS Tag F679). Tick an indicator only when the evidence is in your binder or on your workstation today.
Your saved checklist is locked with the Cognicopia passphrase.
0%
Audit Readiness Score: 0%. 0 of 6 indicators have evidence on file.
The score is a self-assessment of how many of the six indicators you have evidence for. It is not a survey finding, a certification or a prediction of a survey outcome. Your checklist is saved on this computer only, in the encrypted store when the browser allows it.

Copier Calibration & Margin Test
One US Letter sheet that shows whether your copier holds the line weights Cognicopia depends on, and whether a clipboard or binder ring clears the gutter. Print it on each machine, then copy the copy once to see what a second generation does.
The 0.65 in binding gutter the clipboard test checks
Dashed lines: the 0.75 in Cognicopia page gutter and the 0.5 in outer margins
Run the test
Load plain US Letter. Choose Actual size (100%). Turn off “fit to page”, “shrink oversized pages” and any toner-saver or economy mode.
Scale. Measure the two black rulers: the long one is exactly 9.00 in, the short one 6.00 in. If either is off by more than 1/16 in, the machine is rescaling.
Line weights. Find the finest of the five lines (0.5, 1.0, 1.5, 2.5, 3.0 pt) that is solid from end to end. Cognicopia’s early-stage floor is 2.25 pt and its middle-stage floor is 3 pt, so 2.5 pt solid gives a little margin and 3.0 pt matches the middle stage. The finer lines show how much more your machine can hold.
Fine pairs. The groups of parallel lines should stay separate. Merged groups mean the machine is spreading toner.
Black and reverse. The black patch should be dense with no streaks, and the white 14 pt words on it should stay readable.
Gutter. Clip the sheet into your clipboard or binder. The shaded 0.65 in strip should hold the clip or rings, with every word still visible.
Second generation. Copy the sheet, then copy that copy. Compare the 2.5 pt line and the fine pairs.
Marks within about 0.25 in of the paper’s edge may not print on some machines. The test keeps its marks inside 0.5 in for that reason; the punch positions are for reference on screen only.
