Resident Activity Profile

Get to know the person behind the packet

A few short steps about who this resident is: the years they remember best, where they grew up and the work they were proud of. Cognicopia uses it to build activities that feel familiar and respectful.

Privacy First: All resident information entered here is saved safely and strictly on your local computer. No data is ever extracted, collected, or sent to external servers. We ask these specific questions to generate activities that honor the resident's unique identity.

Step 1 of 6: Resident basics

    Resident basics

Why we ask: The name makes every page personal. The decades anchor reminiscence prompts, music and pictures to the years that matter most to this person.

The name they answer to and like to hear. It prints on their pages.

A shorter name or what close family calls them. Used on pocket cards.

Helps staff find the right person in the resident roster. It never prints on the resident's own pages.

Keeps each wing’s residents apart in the Packet Builder: choose a wing there and only its residents are shown.

Shown instead of the name when staff turn on aliases, on screens others can see and on staff pages. Leave it blank to use “Resident” and the room or unit. Never use the resident's name here.

Fill in either one and the other fills itself in. The age may be off by one until their birthday.

    Meaningful decades

The years they talk about most. For many people this is their teens and twenties.

Enter an age or birth year to use this.
